In the Subject line type: NYI Registration
In the text of your email include all of the following information about your gym:

• Gym Name
• Mailing Address
• City, State, Zip
• Gym Phone Number
• Team USAG #
• Contact Person
• Contact Phone Number
• Contact Email Address

Attach a file which contains ALL of the following information for each competing gymnast:

• Female or Male
• Last Name
• First Name
• Level
• Age
• Birthdate
• USAG Number
• T-Shirt Size (Child Med, Child Large, Adult Small, Adult Med, Adult Large, Adult XL)

Note: USAG Number is REQUIRED for each gymnast in USAG Sanctioned Competitions for Levels 4-10.
          USAG Number is NOT required for Unsanctioned Competitions for Levels 2-5

Attach a second file which contains the following information for each of your coaches that will attend the meet:

• First and Last Name
• Level(s) they will coach at this meet
• USAG #
• USAG Safety Expiration Date

Note: Coaches may order T-Shirts at a cost of $8.00 each. Please specify size and include money for coach T-Shirts in your total registration fee.

Mail one check for the total amount of Entry Fees and Extra T-Shirts.
